Travel Pass Application Form
Important notice for applicants. If you are applying for a full academic year Travel Pass (to start on the first day of the Autumn Term) you MUST ensure your application is received by the Local Authority for processing ON or BEFORE the first Monday of June each year. Any applications received after that the deadline date will not be processed until the October of the new academic year and an ‘In Year’ pass will then be issued to eligible pupils. If your child has an Education Health and Care Plan (EHCP) and/or a significant Special Educational Need, you should apply for travel support using the SEND application form which can be found on the website or using the link here  https://forms.sefton.gov.uk/travelpassappformsend If you are the parent /carer of a child looked after, if this child is not looked after by Sefton you should apply to the Local Authority to which they belong. If your child is in college, sixth form or further education, you should contact the college, school or centre and ask about assistance with travel costs. Local Authorities in England have no statutory responsibility to provide post 16 travel support for learners aged 16 – 19 and, as such Sefton Council does not provide any support with travel costs for students. 1. Please check this box to confirm that you and your children are Sefton residents, or in the case of a child looked after, that child is looked after by Sefton.    1.3 Are you the parent/carer of a Child Looked After?
